Person-Centered Planning (PCP) is a values-based approach that places the individual with developmental disabilities at the heart of decision-making. Unlike traditional service models, PCP empowers people to express their goals, define what a “good life” looks like, and choose how support is provided — all in alignment with their strengths, needs, and cultural values.
