Our objective is to support you to gain greater autonomy over your services by educating and facilitating your transition into the Self Determination Program (SDP)
This voluntary program empowers those with developmental disabilities to manage their care with increased flexibility and control over their individualized budget
Rest assured, as a valued client or as the loving parent of a client with the Regional Center, you will not be burdened with any out-of-pocket expenses for our services during your initial transition, 100% is paid by your Regional Center. We are here to support you and your family on this journey, without any financial worry standing in your way.

Orientation Follow-Up:
Ensuring participants understand their rights, responsibilities, and next steps after orientation
IPP-to-PCP Bridge Building
Facilitators review current IPPs and begin crafting person-centered plans that evolve those goals into SDP frameworks
Readiness Assessments
Helping families determine if SDP is a good fit and identifying any pre-enrollment needs.
Spending Plan Strategy
Supporting the first Spending Plan with attention to compliance, creativity, and sustainability
FMS Matching
Helping participants evaluate FMS models and providers based on support needs and administrative capacity.
Our facilitators act as liaisons, smoothing out communication with service coordinators and administrative teams
